Keeping a useful change record
A simple format for recording what changed, why it changed, and how it was checked.
A useful change record is brief but specific. It names the file or service, the reason for the change, and the verification that followed.
The best entries can be read months later without reconstructing the whole incident from memory.
For small systems, a dated note beside the service is often enough. The important part is that it exists before the next maintenance window.
